John Loschiavo trading as Midlife Systems, ABN 56 536 078 003, collects only the information reasonably needed to provide the resource, interest update or service you request.
What is collected
Depending on what you use, this may include your name and email, quiz or guide activity, the limited information you choose to share during a Midlife Health Reset, payment and transaction status, and service messages between you and John. Stripe receives the payment details needed to process the transaction; Midlife Systems does not receive or store your full card number.
The current interest form asks only for your first name, email and consent choices. Do not send medical records, pathology results, medication lists, detailed diagnoses or photographs of injuries.
Why it is collected
Information is used to deliver the quiz, guide, emails or service you request; respond to questions; manage an expression of interest; administer payment and the service; improve Midlife Systems using de-identified patterns; and meet applicable legal, tax, insurance, complaints and record-keeping obligations.
Email choices
Consent to receive information about the Midlife Health Reset is separate from the optional choice to receive broader Midlife Systems resources. Marketing emails identify the sender and include a way to unsubscribe. Withdrawing optional marketing consent does not prevent essential messages about a service you request.
Services used
Midlife Systems currently uses Tally, Kit, Stripe, Google email services, Microsoft cloud storage and OpenAI Sites for relevant form, email, payment, record and website functions. Some providers may process information outside Australia, including in the European Union and United States, under their own privacy and security terms.
General education, not medical care
Midlife Systems provides general wellbeing education, self-reflection and habit-design support. It does not diagnose, treat, prescribe, rehabilitate or replace care from a GP or qualified health professional. It is not monitored for emergencies.
Storage, access and complaints
Reasonable steps are taken to protect information and limit access to systems needed to operate Midlife Systems. Information is retained only as reasonably required for service, legal, tax, insurance, complaints and record-keeping purposes, then deleted or de-identified where reasonably possible.
To request access, correction, withdrawal of optional marketing consent or to make a privacy complaint, email john@midlifesystems.com.au. John will acknowledge complaints promptly and aim to respond within 30 days. Unresolved matters may be raised with the Office of the Australian Information Commissioner or, for Victorian health information, the Health Complaints Commissioner.
